What is a Medical Power of Attorney?
As we age, it is especially important to have discussions with family or friends about healthcare, wills and end-of-life decisions, even though these conversation can be uncomfortable, they are necessary for both you and your loved one.
One legal document that can help ease the pressure on you or your family is a called a Medical Power of Attorney. It is a legal document that names one person the health care agent of another. The person designated can make health care decisions, in addition to speaking to your insurance company, order medical supplies, speak to medical professionals and make any medical appointments on your behalf.
Some medical offices will allow you to ask questions using a HIPAA release form, however, a medical power of attorney will give your loved one a broader clearance. A medical power of attorney will help your loved navigate through your medical insurance company. There are many types of insurances, type A or B pharmaceuticals, home health agencies, and therapists your loved one will need to speak to on a weekly, or at times, on a daily basis. Here is an overview of what is needed to execute a Medical Power of Attorney,
- Download a free Medical Power of Attorney form from your State (see attached) ,
- Read over the document thoroughly with you and your loved one.
- Find a notary and sign with two additional witnesses.
- Provide a copy of the executed power of attorney to all medical offices, medical insurance companies and home health agencies, if applicable.
